1. Please download and print the Request for Funds Form.
2. Fill out the form. Note about your Social Security Number: If you have ever been paid –even once– by Rice University, you do not have to fill out the SS# on this form because Rice already has it. If you have NEVER been paid or reimbursed by Rice, then your SS# is required the first time you request reimbursement.
3. Scotch tape your original receipts on the back of the form. Include a brief explanation under or next to the receipt.
4. Under special instructions, add a note that the reimbursement should be mailed to the addresses listed at the top of the form.
5. Take the form to either of the co-presidents or the club treasurer to collect the necessary signature(s) for authorization to withdraw funds from the club’s account. This is required before the finance department will process the reimbursement.
6. Either drop off your request with Kevin Yuen at the Cohen house or mail it to:
